Out-of-Network Coverage
For out-of-network billing, we will provide a “superbill” to the patient reflecting payment at the Private Pay rate for patient submission to their insurance company for reimbursement. The superbill contains all necessary diagnosis and treatment codes. Larson Mental Health does not bill insurance companies directly for out-of-network claims.
